Challenge
To create an online anonymus survey for employees which gathers data regarding general health. To generate this data in the form of a report for the employer.
Solution
RichardPI created a user friendly application with clear and simple interface. The application requests answers to a set of dynamic questions which were set by the content manager into MS Access database. This in turn generates the health diagnostic report for analysis by Lifequest health expert.
